While oral hygiene, a nutritious diet and regular dental visits are all crucial to long-term oral health, these efforts complement what your body already does to keep your mouth healthy. One of the major players in this function is saliva.

Produced by hundreds of glands located throughout the mouth, saliva does much more than help you swallow and wash away food. As you chew, an enzyme in saliva known as amylase breaks down starches in your food to make it easier to digest in the stomach. Saliva also contains antibodies, similar to what’s in tears, which can fight bacteria and other disease-causing organisms.

Perhaps its most important function, though, is its ability to protect and maintain healthy tooth enamel. The strongest substance in the body, enamel nevertheless has one primary enemy — the acid found in certain foods or as a byproduct of bacteria feeding on sugar and other carbohydrates.

When the ideally neutral pH level of the mouth becomes too acidic (nearly every time you eat), minerals in the enamel begin to soften and dissolve. The increased saliva flow when we eat floods the mouth with buffering agents that neutralize the acid and restore the mouth’s normal pH level. Not only does saliva stop demineralization, but it also restores a good bit of the enamel’s mineral content.

In recent years, a new role for saliva has begun to emerge as a means to diagnose disease. Like blood, urine and other bodily fluids, saliva contains molecules that serve as biological markers for disease. Given the right equipment, saliva has the potential to indicate early signs of cancer (including oral), diabetes and other systemic conditions. As the means to examine saliva for these markers increases it promises to be easier and less expensive to collect and sample than blood, while reducing the chances of transmitting bloodborne diseases to healthcare workers.

It’s a lot to consider with this fluid that you hardly notice, except when it isn’t there. Saliva is proof that our efforts at keeping our mouths healthy cooperate and depend on our bodies’ amazing systems.

While tooth decay and periodontal (gum) disease destroy more teeth than any other causes, both of these diseases are largely preventable with proper oral hygiene and dental treatment. It’s more than possible, then, to enjoy a lifetime of healthy, disease-free teeth.

But even with healthy teeth, the effects of aging will cause tooth wear over time. And although we can’t prevent the aging process from occurring altogether, there are steps we can take not to accelerate the process.

Most tissues, including bone and teeth, have a growth cycle in which older cells are broken down (known as catabolism), removed and replaced by newer cells (anabolism). As we develop during childhood, the growth phase exceeds breakdown; when we reach adulthood, the two phases come into equilibrium. But as we age, breakdown will gradually overtake growth. This aging effect results in, among other outcomes, tooth wear.

“Normal” wear appears to be greatest — and most visible — along the biting surfaces of the teeth. The forces generated when we bite or chew causes enamel to erode over time. Unfortunately, you can accelerate this process through bad oral habits: clenching or grinding teeth, often times at night while you sleep, as well as habitually chewing on hard objects like nails or pencils.

Normal forces generated when we bite or chew are actually beneficial for dental health — they help stimulate bone growth. But when they exceed their normal range as when we clench or grind our teeth, they can increase tooth wear and cause other problems such as diminished function or changes in appearance, such as a shortened facial height.

To slow the rate of wear, it’s important to modify any behaviors that may be contributing to it. In many cases an occlusal night guard worn while you sleep helps prevent teeth clenching. You may also need assistance with stress management, a major trigger for these kinds of habits, through biofeedback therapy or counseling.

If you’ve already encountered excessive wear, bonding techniques using colored composite resin, veneers or crowns that attach directly to the teeth can restore lost function and rejuvenate the appearance and color of your teeth. Most people associate bacteria with disease and ill health. But the real story about the trillions of microscopic organisms now living in and on your body is a bit more complicated. With recent advances in genetic code research scientists are learning that many of these microorganisms you’re hosting are actually beneficial for you — including your teeth and gums.

Beginning at birth and throughout your lifetime you are continually developing a distinct microbiome — actual communities of bacteria and other microorganisms that inhabit your body. As your microbiome develops it helps train your immune system to distinguish between “good” bacteria that help with digestion and other bodily processes and “bad” bacteria that cause disease.  And it continually adapts to changes in what we eat, the pets we acquire or the drugs we take.

But lifestyle choices like diet can also have a detrimental effect, causing harmful bacteria to become dominant. This seems to be the case with Streptococcus mutans, the bacterial strain most associated with tooth decay. Scientists have analyzed biofilm (plaque deposits on teeth) from the pre-industrial era before 1900 and compared it with modern biofilm samples. They’ve found Streptococcus mutans levels to be much higher in modern biofilm, which they directly attribute to the modern Western diet.

As we gain a better understanding of these findings and of the role of bacteria in our lives, it could change many health recommendations not only about diet but about medications too. In the fight against disease, for example, we’ve used antibiotics to eradicate infection-causing microorganisms, but with a broad destructive ability that can also kill many beneficial strains of bacteria. It’s hoped as our knowledge grows we’ll be able to create newer drugs that more narrowly target harmful microorganisms while not affecting beneficial ones.

There’s a new appreciation emerging for bacteria’s role in our lives. As a result efforts to rebalance a person’s microbiome when they become sick may eventually become a critical element in healthcare treatment strategies. The benefits of this strategy for health, including for our teeth and gums, could be quite impressive.

Each June, as we celebrate Father’s Day, we get a chance to pay tribute to the important men in our lives. One of the best ways to do that is by encouraging them to stay healthy—and June is a great time for that, since it’s also Men’s Health Month. So let’s take this opportunity to focus on one important aspect of maintaining good health: preventive dental care.

Preventive care includes all the measures we can take to stop disease before it gets started. One facet of prevention is encouraging people to make healthier lifestyle choices: for example, quitting tobacco, getting more exercise, and improving their diets. You can start by eliminating foods that have added sugar (like many soft drinks and processed foods) or acids (like some fruit juices and sodas, both regular and diet)—and by limiting snacks to around mealtimes, so your saliva has time to neutralize the acids in your mouth that can cause cavities.

There’s increasing evidence that having good oral health promotes better overall health—and coming in for routine checkups is essential. While some men avoid the dental office until they have a problem, that isn’t a wise decision. In fact, a routine dental visit is not only one of the greatest values in preventive health care—it’s also one of the best ways to maintain good oral health. Here’s why:

Tooth decay is among the most common chronic diseases—yet it’s almost 100% preventable! A routine office visit includes an oral exam and a professional cleaning that can help stop tooth decay before it gets started. But when decay is discovered, it’s best to treat it right away, before treatment gets more complex—and costly!

We’ve come a long way since the early 1980s when we first identified the HIV virus. Although approximately 35 million people worldwide (including a million Americans) now have the virus, many are living relatively long and normal lives thanks to advanced antiretroviral drugs.

Still, HIV patients must remain vigilant about their health, especially their oral health.  In fact, problems with the teeth, gums and other oral structures could be a sign the virus has or is moving into the full disease stage, acquired immunodeficiency syndrome (AIDS). That’s why you or a loved one with the virus should maintain regular dental checkups or see your dentist when you notice any oral abnormalities.

One of the most common conditions among HIV-positive patients is a fungal infection called candidiasis (or “thrush”). It may appear first as deep cracks at the corners of the mouth and then appear on the tongue and roof of the mouth as red lesions. The infection may also cause creamy, white patches that leave a reddened or bleeding surface when wiped.

HIV-positive patients may also suffer from reduced salivary flow. Because saliva helps neutralize excess mouth acid after we eat as well as limit bacterial growth, its absence significantly increases the risk of dental disease. One of the most prominent for HIV-positive patients is periodontal (gum) disease, a bacterial infection normally caused by dental plaque.

While gum disease is prevalent among people in general, one particular form is of grave concern to HIV-positive patients. Necrotizing ulcerative periodontitis (NUP) is characterized by spontaneous gum bleeding, ulcerations and a foul odor. The disease itself can cause loosening and eventually loss of teeth, but it’s also notable as a sign of a patient’s deteriorating immune system. The patient should not only undergo dental treatment (including antibiotics), but also see their primary care physician for updates in treating and managing their overall symptoms.

Above all, HIV-positive patients must be extra diligent about oral hygiene, including daily brushing and flossing. Your dentist may also recommend other measures like saliva stimulators or chlorhexidine mouthrinses to reduce the growth of disease-causing bacteria. Together, you should be able to reduce the effects of HIV-induced teeth and gum problems for a healthier mouth and better quality of life.
